Additional Insured Certificate
To add an additional insured, please follow these steps below:
Log into My PHLY and select the policy you would like to update.
Search or select the policy you wish to update.
Select "Add Additional Insured"
Complete the short form.
View, print, or download/email your Certificate of Insurance document
Additional Insured Certificate of Insurance documents can be created online for such entities as landlords, property managers, equipment rental companies, mortgagees, lienholders. If you are hosting a special event, tournament, retreat, or any other type of off-site event you must contact customer service for a quote at 877-438-7459.
