Why do I need a certificate of insurance?

A certificate of insurance (or “COI”) is the standard, official documentation of an insurance policy. Without proof of coverage, many clients will be reluctant or refuse to do business with a company or contractor, afraid that they will become liable for any risks. A COI can show that your company has insurance that protects against workplace accidents, injuries, damages, and discrimination.

How do I request my certificate of insurance?

Insureds requesting a COI must go through their agent – unless requesting a COI for an individual fitness and wellness policy. Fitness trainers may find their COI on their MyPHLY dashboard. Learn more below.

How do I change/create a certificate of insurance?

If you are a policyholder looking to change or create a COI you must reach out to your insurance agent to file a Policy Change Endorsement on your behalf. However, fitness trainers may create their own COI through their MyPHLY account. Find instructions for fitness trainers below.

How do I find my certificate of insurance?
  1. Log into your MyPHLY account.

  2. Select the relevant policy on your dashboard.

  3. Under the Documents section, select "Policy Documents."

  4. Your COI will be located at the end of your policy documents.

How do I create a new certificate of insurance?
  1. Login to your MyPHLY account.

  2. On your dashboard, select your most current liability policy (which will be highlighted blue). This will bring you to the Policy Summary page.

  3. On the Policy Summary page, select the link “Create Additional Insured.” Using the drop down, select the option “Manager, Owner or Lessor of Premises.” Fill in all required fields to complete the certificate and select Submit. This will generate a PDF certificate document.

  4. Navigate back to your dashboard and again select the most current liability policy. This will bring you back to the Policy Summary screen.

  5. Your certificate will be made available under Policy Documents, titled “Endorsement.” In some cases, this can take as little as 30 minutes!

Important note: If you have requested a COI with special wording in the Description of Operations and/or Endorsements, you will need to send in a new request at each renewal.
