Documents Required for Proposal

  • Completed and signed ACORD Application
  • Completed and signed PHLY Water Districts Supplemental Application
  • Completed, signed and currently evaluated Statement of Values (SOV)
  • Currently valued insurance company loss runs for the current policy period and four prior years (Valued within last 6 months)
  • Motor Vehicle Reports (MVRs)
  • Any other Supplemental Applications that may apply (i.e. Dam Supplemental, etc.)
