Fitness & Wellness is a division of Philadelphia Insurance Companies that specializes in providing insurance for health and wellness individuals including professional fitness trainers, pilates instructors, coaches, yoga teachers, dance instructors and martial arts instructors, as well as health & wellness studios that are less than 5,000 square feet including fitness studios, pilates studios, dance studios, day spas yoga studios and martial arts studios.

- Completed and signed/dated PHLY Health and Fitness Club Application
- Completed ACORD applications
- Picture of facility
- Copy of medical disclosure
- Copy of waiver language
- Advertising brochures (if available)
- Insured’s website
- Currently valued insurance company loss runs for the current policy period and three prior years
